Lost / Stolen Badge

Please call the Midway Communications Center at (773) 838-0656 immediately if your badge has been lost or stolen, to cancel access privileges and obtain a confirmation number from the operator.

Replacement of an ID badge requires a completed application marked “Lost or Stolen”, the confirmation number obtained by the Communications Center, and two forms of ID. The mandatory replacement fee for a lost or stolen badge is $50.00; the fee for a subsequent lost or stolen badge within a 24-month period is $100.00. 

Third subsequent Lost or Stolen badge will require a Justification Letter from the company to the Assistant Commissioner of ID Badging Office. The mandatory replacement fees for third lost or stolen badge is $150.00.
Payment of fine does not guarantee replacement of badge.​

Payments can be made in the form of a credit card or money order made payable to the City of Chicago. The Chicago Department of Aviation does not offer refunds on penalties and fines.​​
