Airport ID badge transactions are conducted between the O'Hare or Midway user company signatory and the ID Badging Office, in strict accordance with the Identification Regulations and Practices Handbook. Company signatories are responsible for knowledge and understanding of this directive, for explaining and disseminating these policies and procedures within their companies, and for ensuring compliance of company employees, to include ID badge applicants.
ID Badge Applicants: Contact your company signatory BEFORE visiting or contacting ID Badging Office for guidance and direction in any ID badge transaction. It will save you time and frustration.
Company Signatories: Detailed procedures, for the proper use of these forms, are provided in the Identification Regulations and Practices Handbook. You should be thoroughly familiar with these procedures before attempting a transaction on behalf of your company, particularly, the "Fast-Track System." These forms may be downloaded to your PC, printed and used in accordance with these procedures. These forms may also be "customized" by entering company-specific information (excepting signatures) that is repeated on all such forms your company submits, and copied.
